That would depend on which country you're from. Citizens of some countries are entitled to the visa waiver program.

If you have a british passport you wont need to get one before you fly. Just pack your bags and go its that simple. Once you enter the USA and go through customs they will ask you the usual questions (business or holiday , how long are you staying, where etc). Once that is done they stamp your passport and sort you with a visa for your period of stay.

If your from Ireland or the UK no you don't for 3 mounts if it's longer than that you do need a visa but if your mum or dad are from USA you can get a us passport i know I've a Irish and a us passport

If you are UK resident then you just fill in a visa waiver form that they give to you on the airplane. You hand it in when you arrive along with a customs declaration form. They are fairly straight forward to fill in, but you have to follow a few rules. They usually have a section in the back of the inflight magazine to help you. Make sure you have a machine readable passport.

Not if you're British and have a British passport with at least SIX MONTHS left to run on it when you arrive in the USA. I've been twice to NY in the past 2 years - no probs.

If you have any kind of crimanl record/cautions (even for minor offences) You will need to apply for a visa before you leave.
Otherwise it's just a simple form to fill on the plane.

No I panicked about this mere days before I was going away to the states too. The answer is no you don't, all that happens is that you sign something on the plane before you land. I forget what exactly it is, but it serves as a temporary green card or something and is something we have because of our special relation to the US apparently.

No if you're a citizen of the European Union, Canada, Mexico or Japan. Just your valid passport. Yes for all the others.
