I am anticipating that you are mean a learning plan...

An Individual Learning Plan is based upon several characteristics.

The outline is as follows:

Annual Program Goal(s): You need to have some kind of overall goal in mind of the whole year, or one semester.
In my Psychology class I utilize the following:

The students are expected to understand the basic concepts of sociology, psychology, and anthropology. The students are expected to work independently on a research paper and present it to the class. Weekly chapter tests will determine the student’s current level of achievement for the course, as well as write a research paper, write a final exam, as part of the curriculum.

1. Objectives (Overall expectations)

-(List modified/alternative expectations outlining knowledge and/or skills to be assessed, by reporting period. Identify grade level, where appropriate.)

2. Teaching/Learning Strategies

-(List only those that are particular to the student and specific to the learning expectations.)

3. Assessment Methods

(Identify the assessment method to be used for each learning expectation.) For example, chunking, journalling, modification ....

Don't forget to keep the goals simple!

I hope this helps...

Find out what the deficits are...explore appropriate and particulate ways to fill those deficits, create a timeline with the needful things in sequence, and if you're dealing with a disability, create a reward system to keep them motivated. Although that works well with anyone! :-)

Set yourself some goals, say 3 to 5 years out, where you want to be what you want to be doing. Then look at what you need to do to achieve the goals, do you need certain qualifications or skills, then set about getting them.

You decide what you are going to learn.
Break it down in an outline form how you will go completing that plan--from start to finish. Do not miss any step.
