Never never keep your head bent while doing squats... if you were using a barbell and squating close to your max you would know why immediately. If your head is bent, that means your neck is bent, which means your upper spine is bent, which will make your lower back very prone to rounding. In general your head should be looking straight ahead at all points during the lift, though it is ok to look upwards as well.

Definitely. You want to stick your butt out and use your quads/hamstrings to support your whole body. Putting your head bent over will put too much strain on your lower back. I should know...I threw my back out that way!
