I suffered a head injury during a football training session 5 days ago. I got knocked to the ground and landed pretty heavily on the back on my head. I was pretty dizzy but I could still stand up. However I couldn't remember the fall exactly (brief blackout perhaps). It's been 5 days now and I still get some headaches when I forcefully shake my head. Should I be worried?

If you lost consciousness then you definitely need to go. My son had a similar incident years ago and he was required to sit out half a season.

Here's a good rule of thumb: If you think you might need to see a doctor, chances are you probably do. More often, people don't see a doctor when they need to and serious problems arise. Any head injury serious enough to knock you out should ALWAYS be checked out by a doctor.

Why are you forcefully striking your head?

If you're worried, go to a doctor.
