The website concerns the health issues of the lesbian, gay, bisexual and transgendered community in Scotland.

What it's saying is that within the LGBT community there are varied family structures and parenting arrangements. The site is warning staff not to make the assumption that an adult with a child is automatically the parent. The adult could be an adopted parent, guardian or carer.

Makes sense in any situation to establish the relationship between people before assigning tags such as child, husband, mother etc.
