You can ask your cousin to help you out to process your request by calling the NSO Helpline +(63 2) 737-1111.
1. You have to give your vital information to Teleserv Customer Care Officer. The officer will provide you of your reference number.
2. Pays corresponding amount in Metrobank
3. NSO will process request
4. You will receive application result via courier

Or e-CENSUS www.e-CENSUS.com.ph. For US, Canada and Europe, I think you can pay by credit card (Visa, Mastercard, AMEX etc.)

The NSO has several sections/divisions. Although your question is rather vague, the NSO Legal Section might be able to help. The landline numbers are :7137063; Telefax 7160734; fax 7137073; staff # 7160807 & 7141723.

Errr.... you din't say if you were born in the Philippines. If so, the the main government portal web page is:

http://www.gov.ph/
Then click on "how do I get a copy of my birth certificate?" (sorry, but Yahoo won't allow the whole address) Anyway, there you can find all the information, requirements, and fone numbers.

If, however, you were born somewhere else, NSO wouldn't have a record of your birth. You'd have to show the birth certificate from where you were born.

There's an NSO to Manila City hall or Pasay near DFA. Or we do arrange also for documentation at P390/copy.
