Generally the rule of thumb is 15% to 20%.Some people adjust that depending on the food and serviced received.Large parties the gratuity is included at 18%.So with a bill of $100 the answere would be 15 to 20 dollars.If your server did something special or you had an extraordinary dinner feel free to tip extra if you can.

Well you are suppose to tip with the tax included. But, I'd say the least you should leave is $15-that's 15%. But, personally I would leave $20-that's 20%.

If the service was horrible you could try to leave less -- or no tip -- but that would be really rude on your part. Bad service needs to be brought to the attention of the management. If the service was good and you feel real generous you could leave more.
