Physically: getting stabbed in the heart hurts most.
Emotionally: Getting stabbed in the back because it is the ultimate betrayal and most hurtful action.

Question dilemma....do you mean physically with a knife in the heart or back? Or do you mean physiologically given a stab in the heart(heartbreak) or stab in the back (being stabbed vocally)???
WITH A KNIFE? in the back...the knife blade may miss the heart
and/or other organs.
A knife wound in the heart? You be dead!
Heartache or backache? Backache anytime....easier to get over than a heartbreak!

for death-the heart would kill you while the back MIGHT not
