I need to figure out the best way for me to approach one of my coworker's on her dress attire for work. She is in her mid 20's but has the face of a teenager and while her dress is appropriate for our office, I feel that she can look better when she goes to clients and works directly with them, thus representing our company. She normally wears a pair of slacks and a blouse or sweater. How can I approach her and what do I recommend she does to improve her appearance, thus giving her some more credibilty with our clients?

That's her supervisor's job, not yours.

I would talk with your H.R. department and involve them in the problem. Get their advice on what has been done with other employees in similar circumstances. If you have a dress code policy that is given to your employees sit her down and review it with her. Hopefully there is something in the policy about the manner of dress for those who work with clients directly, or go out and visit clients. Remind her that she is the representative of the company and the image that the customer see's. Perhaps ask that she wear a dress or skirt when visiting clients.

IF you are not her supervisor it is not your responsibility to critique her wardrobe. Whats wrong with dress slacks and a blouse or sweater?
