Is this awkward sentence grammatically correct: Mr. Jones, with the service I have provided you today, are you very satisfied?

Mr. Jones are you completely satisfied with the services you have been provided by me today ?

I would rearrange the sentence so it reads
Mr. Jones, are you satisfied with services I have provided for you today?
If you wanted to be formal I would use the following sentence:
Mr. Jones, are you satisfied with services you have received today?

No, put Mr. Jones with are you satisfied?

An infinitive is to plus a verb---to go, to walk, etc.

"Mr Jones, are you very satisfied with the service you have received to day" would be better.

A split infinitive would be like this: To gladly go (should be to go gladly).
