Have you tried just "admin" for both username and password.

Otherwise try

Username: D-Link Password: D-Link

Or

Username: admin
Password: (blank)
(blank) literally means leave it blank.

There should be a reset button at the back of the router. You could keep that button depressed for about ten seconds while the router is powered on and this would bring the router back to factory defaults. Once in factory defaults, the router will accept the factory default password provided in your user manual.
