I've had a Queen size bed for about 8-9 years. The mattress has started sagging in the middle, regardless of how we rotate it. I think it's time for a new one! But the box spring underneath seems fine. Do I need to replace that too?

The mattress company's say that you should but I didn't when we bought a new mattress a month ago and everything seems fine.

If you get a new mattress of the same size, you don't need to replace the box spring.

However, you should definitely check to make sure the box spring isn't also sagging in the center.

i would say no. theres a box spring that is about 12 years old in my house with a new bed on it. see over time the box spring might lose support, but it all depends on how you like sleeping, do you like the feel now, because a new box spring might be alot firmer and supportive.

Only if it too makes noises. Or if you want colors to match but you can use a skirt for that but to answer your question "NO".
