I believe it was episode 6 of the 3rd season. I'll go check and give you the link to the plot summary.

Technically, she didn't ask if I remember right. While talking to him in his glass cage, she showed him a note that told him to kill Ben during the operation.
I'll defer to the others for the exact episode name and/or number.
