You have the chain from the handle lever to the flapper to long. Adjust the chain shorter until the flapper stays when you flush the toilet.

The thing that makes the flapper close is suction, that is, water leaving the tank fast enough. So, if the jet (the one inch hole at the bottom inside of the bowl) is clogged with calcium deposit, that will affect it. Turn off the water supply, flush the tank to empty it and fill it with a gallon of vinegar. Let soak for several hours. Check to see that the jet is clear, it should be open enough for you to fit your index finger through. (really) You may need to "chisel gently" at the calcium with an old chopstick or screwdriver. I guarantee that this is the problem.
