A stack of fat quarters
A subscription to quilting magazines
gift certificates to quilt shops or fabric stores

Ask if there is a book, pattern, or ruler they've been wanting and get it for them.

I agree with Jarien that a gift certificate to a favorite fabric shop would be the best thing to give. Only the quilter knows what she really wants, or needs.

A good rotary cutter with blades
A cutting pad with markings
a variety of Quilters Rulers
a variety of Quilters templates
subscriptions to a quilters magazine
a couple of stacks of "fat Quarters" in interesting patterns
Really good scissors

As a quilter, I enjoy fabric shopping. A gift certificate to a local fabric store is always going to be appropriate!

Other ideas might be a storage box for projects, an excellent pair of scissors, a new rotary cutter with extra blades, a cutting table with adjustable height.... Does your quilter use a sewing machine? Offer to take it in for a cleaning and service!

There are CDs that you can buy for him/her. I found some online (sorry, I dont quilt so it's hard to listen to quilter's music). Cathy Miller and Ricky Tims have CDs that are good for quilting (so I've heard). Tims plays the piano.

You can also give him/her a nice big bag to carry all their supplies in. The last link has funny gits liek underwear, hats and t-shirts with clever comments.

Quilter's also like jewelry (or even home decor**) items with quilt themes on them... could be block patterns, whole quilt patterns, or even images of rotary cutters, thread, etc. These could be made from polymer clay, decoupaged, or many other things.

**pencil cups, candle holders, stationary, trivets, containers, etc.

They have these cool needle threaders, I think that's what a quilter would want!

Kai Scissors. They are far and away the best scissor and the price doesn't break the bank.
