Condoms by use are only 86% efficient. That means that 14 out of 100 people who use them get pregnant. Buy some spermicidal foam to go along with them. That helps a lot.

But I haven't used condoms in years

Good Luck try to use the spermicidal ones and maybe make sure shes on the pill
