No. Chancellor was originally ugly! He had been using the force to disguise himself. His force lightning had reversed the effect and shown his true form. In a book, han solo once saw the Emperor's clones. He said it was ugly the moment it was born

It was the force lightning that gave him his look. After the attack from Windu, he makes a public announcement regarding the Jedi's and how they should be wiped out and due to a recent attack from a Jedi, he has become disfigured. I don't know the exact quote but it went something like that.

The chancellor was forced to envelop the "evil" he was projecting at Windo, because Windo was creating a feedback of the energy. The evil energy disfigured the chancellor.

I think its his true appearance. It wa is full force, manage to blow Windu a mile away, unlike Luke, who manage to stand it in episode 6.

I think the Froce Lightning was the cause. He was using an awful lot of power and I think his disfigurement was a side effect of that.
