how do you report a company to trading standards for major health and saftey breaches?

to contact trading standards in the UK people should now go though consumer direct http://www.consumerdirect.gov.uk/ brought together by DTi and Office of Fair Trading.
however due to the issue you have detailed i would go directly to the Health & Safety Executive http://www.hse.gov.uk/contact/index.htm HSE Infoline 0845 345 0055

Just give then a call and someone will take down details of your complaint the number should be in the yellow pages or look on your local council website
