It's a song about friendship and love. There are two characters: One who is portrayed by the singer and the other who is a friend, or a romantic relation.
It's a testimony of support, saying that you shouldn't get discouraged in times of trouble, and you're strong enough to deal with anything. The singer backs this up by saying that he/she can see the subjects "True Colors", and that's why he/she loves the person so.
When you feel ugly, when you feel down, when you feel unloved, the singer is saying he/she will always be supportive because he/she sees the true beauty of the subject of the song.
It's one of my favorites!
