123 E. 34th St., Apt. 1 is grammatically correct.
On an envelope you could put Apt. 1 under the street address.

I'm not positive, but to me the comma just looks right. I was thinking that the apartment number went below the street address, though.

In a document, you should include the comma, but on an envelope, it is not needed.

I am an xpostal worker, it doesn't really matter. Most post carriers don't even look at it. If you were writting an essay then yes you would.
