It depends on the kind of lawsuit (civil, small claims, etc).
First get a lawyer.
If you can't get a lawyer, visit your courthouse and ask the clerk what is necessary - it varies from city to city and state to state, and who might have jurisdiction. The clerks can be very helpful in giving you the proper forms to fill out.
For small claims (under $2500, usually) you can file in Small Claims court for about $25 at your local courthouse. If it's a lawsuit for something else, and you can't afford a lawyer, the clerks will point you in the right direction as to forms.

If it has to do with employment such as some kind of discrimination, wrongful termination, etc, then you have to file a complaint with EEOC or DFEH. You cant sue until you do that. They have to issue you a right to sue letter first. Then you have to get a lawyer to file your lawsuit for you.

Much also depends on the issue. Car accident? Medical malpractice?
