I don't really believe it that much. From what I've heard, if a guy wears regular underwear (tighty whities), they are restricting. I guess they pull his balls up and into each other and they got hotter that way. Then his sperm gets hot and if the sperm gets too hot, it's not that potent. Boxers let his balls hang looser and they don't get as hot and sweaty. However, I think if a man has healthy sperm and a high enough sperm count, wearing tighty whities isn't going to be that big a deal.

It depends on his sperm count, and the only way to know about that is to have a semen analysis done, which a doctor can prescribe. The testicles are outside the rest of the body because they prefer to be at a lower temperature than the rest of the body for optimal sperm production, and boxer shorts help to keep the testicles held away from the body and cool rather than close to it, and warm.

He doesn't HAVE to. It will help get his sperm count up if it is low and you're having trouble ttc. If a guy's sperm count is low, he should also avoid alcohol, raw seafood & hot baths.

If the man has a low or marginal sperm count then it is absolutely necessary!! Otherwise, I don't think it matters much.
