Actually there are quite a few, but, most are in high-limit rooms...
Bellagio has two in the baccarat room, at least one is open 24 hours.
Caesara Palace also has two, one on the main casino floor the other in the high limit area
Golden Nugget - one in the high-limit area
LV Hilton - one in the high-limit area
Luxor - one in the high-limit area
Mandalay Bay - has two, one in the Baccarat room & one on the main casino floor that's open on the weekends only.
MGM - has two, one on the main casino floor, one is in the Mansion.
Mirage - has two, one on the main casino floor that's open 24 hours, and one in high-limit.
Paris - one in high-limit
Stratosphere - one on the main floor, open 24 hours, this one is your best shot at low limit play, I have seen it at $5 on weekdays.
Venetian - has two, one on the main casino floor open 24 hours, & one in the baccarat room
Wynn - has two in the high-limit area

The Wizard of Odds site has a list of casinos with single zero games. I can't vouch for them.
I can say that I have seen them on the main casino floor in Monte Carlo (the Vegas casino, not the city in Monaco), next to double zero wheels. The minimums were typically twice that of the 0/00 wheels.
I have never seen $5 min. at any of the big strip properties. The Monte Carlo game was $10 min, because the double zero games were $5.

In every casino, it's not hard to find. No they play double zero roulette in America. You can play any denomination you wish, they will just change your chips to that amount.
