That's a good question, I've been thinking that one over myself. After watching every episode, I'm pretty sure that only Brian can understand what Stewie is saying. I don't believe Lois is ignoring him, I think she's only hearing him baby-talk.

You know, in the first season it almost seemed like they could. He would talk, and they would answer as if they understood exactly what he was saying. After that, he'd be talking, and no one would really pay attention, or they thought the wanted to eat or something. It's like they only hear him talking typical baby babble. The only one I know for sure totally understands him is Brian.

Only Louis and Brian can hear him, Everyone else just tries to figure out what the heck he is saying

the only one who hears him is the dog. and lois.. but she ignores him and chalks it up as baby talk.. awww how cute..
