This is a psychological problem that needs a good analyst; but tell him to try a routine:

When he goes to school, or work, wherever, tell him to immediately begin looking at people he is passing, or who are passing him. Look at each one individually. Say to himself: That is a unique person. That person has value. That person deserves attention.

Always refer to everyone around him, and do this for two days.

After awhile he will get into the habit of thinking of someone else.

If he sees someone struggling to open a door; open it for them and ask if they need help.

If he is in a store and a person looks bewildered, as if he can help THEM.

Is he an only child?
Is he a middle child?
is he a youngest child?

there are so many reason we crave attention; and it usually stems back to the fact we got none when we needed it most.
