Don't approach them - if you have concerns you should talk to your supervisor. Make sure that you have VERY specific examples of where they fall short.
If there are other people in your workplace that feel the same way, have them come foreward too.
If no one else feels the same way, you may want to re-examine whether this person is actually under-performing, or whether you have a presonal problem with them that makes their work ethic annoying to you.

If is not your job then you probably should not.
I am on our faculty eval committee so in addtion to all of the other things I must do, teach, research, write articles, advise students, faculty advisor to a student group, and search committee - we have to evaluate faculty every year. We are trained to do this and our views carry some weight.
EDIT
But if you are doing this as a friend ask if there is a problem. State that you don't see the same energy that she/he used to put into their job, and you wonder if something is wrong.
