I think that somehow the Doctor will be conveniently granted a few extra regenerations; perhaps as the last Time Lord he will have "inherited" the regenerations of all the dead Time Lords. Or, there were a couple of hints, in the time of Sylvester McCoy, that the Doctor is a re-embodiment of Rassilon, so maybe the writers at the time will somehow weave that back into the story, giving the Doctor an unlimited number of regenerations?
Basically, Doctor Who is too popular to die, there's simply GOT to be a twist! Or at least, as a dedicated fan, that's what I tell myself!

When the 11th Doctor's story goes on, it will revealed that he is in fact the 13th incarnation. Because of this, he will live into ancient old age and nearly die, but the Time Lords will give him a new regeneration cycle to save his life.

Willie 1005 have you forgotten about Peter Cushing. Paul McGann isn't the only Big Screen Doctor. Technically David is the 11th Doctor so there is only 2 more regenerations. I for one am glad they brought back the Doctor, it's about time there was something half decent to watch on telly. So I hope there will be an unexpected twist somewhere along the line. Maybe the Doctor will turn out to be a robot once his 13th persona fizzles out. Maybe he's not the last of the time lords. Russell T can pretty much make up any kind of story to keep it going. If he can pull off bringing back the Daleks, Cybermen and K9 he can do anything. Long live Dr Who!

There has never been a definitive pronouncement in the show itself that says a time lord is limited to 13 regenerations, as far as I know.
The new series has not felt bound by existing continuity, in any case - witness the retconning of the Dalek's leader etc. And if they really wanted to, it wouldn't be difficult to work out a way of extending the number arbitrarily even within continuity - some handwaving with a new monster or gizmo that has an unexpected effect on the Doctor would do it.

There *have been* 11 medical doctors (so far) and 10 regenerations. Your chum might have been thinking of the theory that factor Lords *get* 12 regenerations entire, meaning 13 diverse medical doctors in all.

The Doctor has regenerated nine times. He has another four to go. I'm sure that by the time his "last" incarnation (13th) comes along, the writers at the time will have thought of something.
I think the Eye of Harmony or the Matrix will have something to do with it.

They will probably come up with some sort of unexpected twist like they always do, that way they can change the storylines to fit in with any situation.

I've heard rumours of a son all over the Outpost Gallifrey(#1 site for DW stuff) forums. That could be how.... but a twist would be nice. The whole "son" thing is just cheap.

Sadly, I think they have forgotten to much of the doctor's past. The writting is going to get muddled and filled with retcons
