Both are acceptable, depending upon the meaning sought to be conveyed.
An aide is an assistant, and so first aide would be the first assistant if there are many assistants.
First aider is one who comes first to help or one who provides first aid.

First Aider is a person who knows first aid.
First Aid means immediate treatment
