With most patients they find out they are carrying twins during an ultrasound during the first trimester.
If they are having some kind of fertility treatment they will find out fairly soon after conception since ultrasound exams are usually carried out more routinely very soon after a pregnancy is confirmed.
If patients are having problems, or have a history of problems such as ectopic pregnancy, or are not sure exactly what their approximate date of conception is then an early ultrasound will also be performed which should let them know two or more are on the way.
Patients who are having otherwise "normal" pregnancy usually find out when they have their nuchal translucency ultrasound exam at around week 11.
Another sign to cause a doctor or midwife to suspect a multiple pregnancy is a larger than average weight gain during the first trimester. Or even if a doctor or midwife thinks they can hear two heartbeats on the doppler.
Pathologically measured high hormone levels or a larger than average abdomen are not reliable indicators of a multiple pregnancy, however many women who turn out to be expecting more than one baby per pregnancy do seem to report more severe early pregnancy symptoms such as nausea, breast tenderness and frequent urination, although this evidence is anecdotal rather than pathological.
There are many myths about twin or multiple pregnancies that cause patients to demand you check to see whether thay are carrying twins - the first is usually an alleged "family history" of twins (in reality most twin pregnancies are either spontaneous, or have more to do with advanced maternal age or fertilit treatments than any type of family history - and twins do not "skip generations", that is the oldest wives tale in the book, I hear it all the time!) and the second is because everyone is telling them they are "huge" and must therefore be carrying twins.
The only way to 100% confirm you are having twins, despite any other signs, is by ultrasound exam.

Your doc can do a blood test sweetie and if the count is high they know it is twins. Otherwise they can also do an ultrasound to find out if there is two heartbeats or just one!! Also if twins are in your family it usually skips one generation and if you are over 35 the chances are also much greater for multiples or if u used any medication to help you get pregnant...Those are all of the clues but there is no real signs except for those that can be done through your doc!! Soooooo dont be scared just ask them to do a blood test if it is still really early or a scan... Good luck all the best!! - If u are having twins Im gonna be really jealous!!!!!

When you have an ultrasound, there will be two blobs with heart beats.
Thats how we found out we were having twins.
