The Doctor Destiny I recall used some sort of device to transform his thoughts into reality; while Nightmare was actually an other dimensional being who had that power as an inherent ability. So, unless they have made changes to DD that I’m not aware of, I have to go with inherent power over a device.

Depends upon where the fight took place. Nightmare would be powerless unless they met up in his domain, in which case, he'd dominate.
