Literally? To "screw" means to have sex, and not in a good way (it is a more socially acceptable way of saying "f-u-c-k you").

If someone "screws you over", it implies that they played a mean trick on you, or they pulled a con job on you that left you in a bad position (similar to when a bad guy uses a woman for sex, and then leaves her pregnant and alone to raise her child by herself).

Usually when someone says this, they are hoping that someone will use you and leave you without recourse. They are hoping that something bad will happen to you.

------------edit------------

If someone says "you're screwed", he means that you are in trouble (it literally means "you are pregnant"). It usually means that you have been taken advantage of and abandoned.

It means that you do not care about this person's opinion. You think that they are being unreasonable, bossy, unfair, etc., and you deceide to rebel.
