A gift to the manager depends totally on the factors of:
1. Occasion,
2. Relationship with the manager (direct report or just a manager from other department who is known to you),
3. Closeness and understanding with your manager (whether too formal or too friendly),
4. Expectations of the manager,
5. Your convenience and budget.

As aforementioned, your convenience comes last. Always buy a gift which will be useful to the manager and fits perfectly in your budget. The gift should not be embarrassing for both the giver and the receiver. For most occasions, a card (thank you, get well) or a memento (in case of good hike or promotion) will do.

Also, make sure to wrap the gift neatly (after removing the price tags) and present it humbly without creating a fuss of presenting. Humility has to be maintained at the right level.

Depends on the situation. Is the manager getting married? Birthday?

A small gift for a certain occasion may be appropriate if you work closely together. It should not appear that you are sucking up.
