I am writing an essay to an honor society (some student I am) and I can't figure out if I put my name on the top left or right side. Also, should I put the date? Ah! Help! Someone!

In a formal essay, the heading always goes like this:

Name
Subject
Teacher
Date
Topic (centered, underlined, one line after the heading)

===> Start your essay after a tab

Example:

Jane Doe
English
Mr. Doe
2/20/07
Yahoo Answers (centered, underlined)

I'd go with something like the MLA format. (But read all over and see if the honor society has given you a format or an example.) The MLA would have you start one inch down and one inch in (just set margins) and on double spaced lines write your name, the society,
the assignment, the date. See below.

Sally Student

National Honor Society

Formal essay

20 February 2007 (Yes, the date is notated this way.)

If they didn't tell you how they wanted it identified, you are pretty sharp to go asking. Blessings on you.

Formal essays come with a cover sheet. This includes the title at the top, the name (with "by") halfway down the page and any other pertinent information towards the bottom.

Also, your last name should be on the top right hand corner of every page along with the page number.

Just like the others have stated it depends on the format, if you have to use APA format it goes like this:
Title of Essay
Name of School
Course Name; Course Number
Instructor Name (and title if appropriate)
Date

Good luck, hope this helps.
Almost forgot.....this information is centered in the top half of the page and double spaced.

Not necessarily. You can if you wish. In lower case it would be acceptable since that's the way he published it. He never used capital letters in his own name. I had to do a similar assignment in a poetry class in college. Prof actually preferred the lower case letters since that's the way the author always signed and published his name. I got an A on the paper.

It depends on the format that they require. If there is no standard format, put your name and the date on a cover page. It will look more professional.
