Write it however you want, unless you want to be a stickler for etiquette. I personally would go with:
To: Cody, Elias & James
From: Thalia
But thats just me.

Use their names and sign it Love, Thalia - if you aren't close and there is a chance they won't know who you are then sign it Love, your cousin Thalia.
