For your first question, it is actually called a "Double Triple Double". The only player to ever do it was Wilt Chamberlain. On February 4, 1968 he had 22 points, 25 rebounds, and 21 assists.
For your second question, steals and blocks were not kept as an official stat until the 1973-1974 season. Wilt Chamberlain most likely had a few "unofficial" quadruple doubles, but because steals and blocks weren't kept until the season after he retired, he doesn't get credit for them. Officially, there have only been four players to accomplish a quadruple double:
Nate Thurmond - October 18, 1974, 22 pts, 14 rebs, 13 assts, 12 blks
Alvin Robertson - February 18, 1986, 20 pts, 11 rebs, 10 assts, 10 stls
Hakeem Olajuwon, March 29, 1990 18pts, 16 rebs, 10 assts, 11 blks
David Robinson, Feb 17, 1994, 34 pts, 10 rebs, 10 assts, 10 blks
