Congo! The book was amazing and the movie just plain sucked.
I also want to say Sphere. I never saw the movie because I heard from lots of people that it was horrible but the book rocked and I want it to stay that way for me.

The Odyssey was a horrible movie. Horrible acting, effects, translation of the story from book to screen, etc.
Oh, gosh, but even worse was Ella Enchanted. While I admit the movie is kind of cute, it was so different from the book (which I have read many times) that I didn't even know how the movie would end when I was watching it. They should NOT have given that movie the same title as the book, because it was and ENTIRELY different story!

The Da Vinci code was a pretty poor adaptation. The Sophi Character was too much of a damsel in distress in the movie, in the book she took a much bigger part in breaking the codes. And it was silly to change the relationship between her and her grandfather, it served no purpose. Too many unnecessary plot points were changed.

"Midnight in the Garden of Good & Evil", by John Berendt . The book was great ; but the movie left much to be desired . The only thing worth viewing the movie for , is the scenery in Savannah , Georgia where the true story took place .

"The Hitch Hiker's Guide to the Galaxy." I don't think they even read the book. They probably heard some people talking about it in the next cubicle and got it wrong.
A close second is "The Firm". The credits should have said "Very loosely inspired by some of the characters and situations in the novel by John Grisham" instead of "Based on..."

The Shining!
Amazingly, Stephen King was disappointed in Stanley Kubrick's version. (One of my favorite movies of all time), and re-made it in the 90's starring one of the brothers from WINGS! "Tony", Danny's imaginary friend, was actually this cheap superimposed computer effect, rather than Danny's finger. Acting was crap!
Also, To Have and Have not....Ernest Hemmingway's blood-soaked tale of a mutilated, cold-blooded rum smuggling, human trafficker, was BUTCHERED in the Humphrey Bogart / Lauren Bacall film version. Sort of like turning The Texas Chainsaw Massacre into You've Got Mail.

The Virgin Suicides. i became so so so mad, no longer a lot that they made the ladies blondes as a replace of brunettes, yet because of Kirstin Dunst turning out to be the numerous concentration, which isn't what's became about. actually terrrible And my husband hates Starship squaddies, they ruined a military e book and made it a sci-fi crapfest.

No film is ever as good as the book
I'm a big Stephen King fan and, apart from Misery, the film studios change loads of stuff in the film versions of his books which totally ruins them for me (especially when they change the ending)
