I watched a show on tv today that brought to mind a good question. When someone has a sex change, do they get a death certificate for the person they changed from? Then turn around and get a birth certificate for the person they changed into? Or do they have the original professionally fixed to where they can change their name, gender? How does this work?

No death certificates, no. Nor new birth certificates.

Where changing your sex is legal and recognized, you can go to the courts and petition to have your sex legally changed on your documents, like your drivers' license and other documents. Your new sex will be officially seen. All you have to do is prove that you're transitioning your sex (have had gender therapy counseling, a recommendation, and plans to pursue surgery, etc.). But again, this is only in places where it's legal (in a lot, it's still not- they won't change your sex in the eyes of the law).

Yeah i saw it on a show too. I do think they put the new gender on the death certificate, but the birth cirtificate stays the same.

You leave the birth certificate alone. It is a historical record. If you change your name, you might want to make it official in court. No one thinks you are a different person.

I've heard that people change their birth certificate to reflect their new name and gender, but some jurisdictions may not allow it.
