In "Welcome to The Black Parade", he talks about his father taking him to see the parade and saying that he hopes that someday his son will join them.

The entire intro is about his father, so why in the first sentece, does he say,

"Sometimes i get a feeling, she's watching over me."

okay.
so this whole album is based on a character called "the patient"
what Gerard means by the parade is, Gerard believes that when you die, death comes for you however you want it too, so that is basically the theme of the album.
so, the patients strongest memory is when his father took him to see a parade in the city, so death comes to him it the form of "the black parade". the "she" he is referring to is "mother war" another main character on album.
hes not saying that "he hopes that someday his son will join them"
hes saying that we will all die eventually.

I am guessing that Gerard is reffering to something or someone different. the album is about, dying, and being taken by death in the form of a Black Parade. So I would assume that it is refering to death.

I assumed he meant his grandmother "helena" was watching over him since shes dead and all.

or

hes talking about a girl that is with him at the hospital which he will leave her a "weary widow" so throughout the song he does mention she not he. just in the beginning where hes remembering his father taking him to a parade.

I'm pretty sure it is about Elena, his grandmother that passed away. Because she's a big part of his life and the band starting off.

It could be gerard talking about his grandma? Or, they said she gets mentioned a lot in an interview, some random girl.

Maybe the she part is referencing to his mother..
or maybe it just worked better with the word she instead of he so they changed it.
