Depends on what your trying to do, a birth certificate works, if you have not got that yet, then photo copy the proof of registration. If all else fails, seek legal advice.

I am trying to figure out why you would need that in the first place. Are you in the US? if in us then go to a bank and show ask for a notary public. Show the notary all your papers on your baby and ask them to help you work a affidavit of proof of baby's identity. If you are elsewhere sorry I dont know how it would work there.

I just did a simple search "AFFIDAVIT OF IDENTITY" and it seems to be relatively simple.
You might try something along the lines of:
This Affidavit is to establish and verify that (child's name) is.....
since an affidavit is notarized i think you will actually go to someone who is a notary public (find one in a phone directory) and they might even have a basic form you can fill out and sign, which they will then sign and notarize for you. it should not cost much either, i would guess up to ten dollars.. ... good luck.

The document has to be legally notarized. You can do this at a bank your financial institution will usually do it for free or a very small fee. Also, it has to be wrote in standard legal format. So, just typing up something won't work at all. Especially if it's for a government application, like a passport or visa.

I can't be of further help until I know for sure why you need this letter. I'm assuming for a government purpose...and this was the only thing I could think of.

I'm pretty sure you just write out the date and place where the child was born, then take it to a bank and have it notarized by a notary public.

They want the same information that is on the birth certificate, just wriiten and signed by you. Notary is optional. You should ask if it is required. Basically,
Name of child
Birthdate
Place of birth
Mother
Father
Current address of child
Social security number (if you have one)
I was recently asked for this for my kids, this is exactly what they said they wanted.(notary was not required)

It would need to be a legally notorized document. Depending what you are using it for, they may have a template you could fill in. You may want to think about consulting a lawyer.
