I am desperate, I have no clue how to write it. I have a three weeks old baby and I need to write affidavit of proof of his identity. Could somebody give me a link to a website or give me an example of what exactly their should be written? I am not american, English is my second language, so I am really having a hard time figuring those things out.

I just did a simple search "AFFIDAVIT OF IDENTITY" and it seems to be relatively simple. You might try something along the lines of: This Affidavit is to establish and verify that (child's name) is..... since an affidavit is notarized i think you will actually go to someone who is a notary public (find one in a phone directory) and they might even have a basic form you can fill out and sign, which they will then sign and notarize for you. it should not cost much either, i would guess up to ten dollars.. ... good luck. xxoo

I am trying to figure out why you would need that in the first place. Are you in the US? if in us then go to a bank and show ask for a notary public. Show the notary all your papers on your baby and ask them to help you work a affidavit of proof of baby's identity. If you are elsewhere sorry I dont know how it would work there.
