This is a tricky question-- I'm guessing that by literacy you mean reading. There are all sorts of other kinds of literacy: information, media, health, computer, etc.
But as far as reading, writing and spelling are concerned, 20 countries are tied for 99.9% literate populations. They are: Australia, Austria, Belgium, Canada, Czech Republic, Denmark, Finland, France, Georgia, Germany, Iceland, Ireland, Japan, Luxembourg, Netherlands, New Zealand, Norway, Sweden, Switzerland, Trinidad and Tobago, and United Kingdom.

All of the contries that I am about to list have a literacy rate of 99.9% and are ranked #1 Australia, Austria, Belgium, Canada, Czech Republic, Denmark, Finland,France,Georgia,Germany,Iceland,Ireland,Japan,Luxembourg,Netherlands, New Zealand,Norway,Sweden, Switzerland,Trinidad and Tobago and Last but not least United Kingdom.

Of the countries that have literacy rates at or near 100% Sweden has the smallest percentage that are at low level of proficiency 25%, For comparison the in the UK and the US it is about 50%.
