I live in West Virginia. My son was born 3 months ago, and I have his social security card, but I do not have a birth certificate for him. How do I get it?

Also, how much money would it cost to change his middle name? And would I need the birth certificate in order to do that, or could I just use the social security card?

Any information would be helpful!!

The cost of a duplicate certificate is $10 if you never received the original call and ask if it is usually this late and if you can request it without cost since you did not receive the first one their address and phone # are:

You can also ask them about changing the middle name prior to receipt (it may not be too late) as well as costs to change it if it is already registered (usually under $200). Good Luck!

call vidal statistics. if i remember it's only 5 or 6 dollars. they will send you a copy.
