born with them. The placement and shape of the ears is decided in the womb. once the cartilage hardens as the months and years pass, thats their place.
Some moms used to tape their babies ears to their heads to make them flat.
Lol.
Its really not that big of an issue. Look at Will Smith. Didnt ruin his life at all.

My SIL swears it is from sleeping and having your baby's ear bent at feedings, etc., and is EXTREMELY conciencious about never having her kids ears bent (or anyone else's kids ears bent for that matter). Even so, two of her three kids have ears that stick out big time...so I'm with the others....its all about genetics.
