I can only give you English hints, but perhaps it will work the same way in other countries.
Ask at your local health centre!
Contact your local Primary Care Trust (health authority) - they are always carrying out surveys on these sorts of things.
If you are doing it for America (as some of the answers suggest!), try emailing a British Primary Care Trust (put Primary Care Trust into a search engine and you will get a list of them) and asking them for what they have!
