Have a look in the "Contacts" or "Phonebook" area and looking for "Copy contacts to simcard" or something along those lines.
Select all contacts and transfer.
All contacts should be stored on the simcard so when you take it out of your phone none are left behind.
With that said some simcards only allow a certain amount of contact saved to the simcard.

Yes, but it is better if you do it yourself. There are plenty of Cellphone Phone Book transfer programs out there that support multiple phone types from different vendors. The "SIM card" phone book transfer usually doesnt work with most people - who just puts in a phone number and a first name anymore? That is all that is stored in the SIM card... I have names, addresses, emails, pictures and voice activation commands associated with them. So an external program is great to have.
If the phones come from the same manufacturer then the same software MAY be usable between phones and you may be able to transfer the numbers that way.

Depends on the model of your old one. The new phones have a removable chip that will transfer your phone book when you switch phones.

yes most places where you buy your new phone got a handy little gadget that they put your PHONE MEMORY card!! into, and "hey presto" its done. Ringtones phoneoo's the lot and yhey wont chargwe you for it!! had it done myself a couple of times!!
