The phrase was coined by the Nazi's during WW2.

It refers to there actions towards the Jews in Germany at the time.
The final solution being the extermination of all Jews.

the term originated, in its largest context, the determination by the german government's official policy of annihilation of the ENTIRE jewish race in europe. it was discussed at the wanasee conference in 1941, by members of the german government, the german military, the SS, and was called to order by ss colonel heydrich, offically recorded, then the written records were edited and distributed to the various ministries, by ss lt. col. adolph eichman, who implemented it as the administrative authority and offficial responsible for its full and complete implentation under SS authority. heydrich was assainated in czechoslovakia in 1942, and eichman, having escaped to south america after the war, was kidnapped, taken to israel and tried as a war criminal, convicted and hung by the israeli government. his defence was "i was only following orders", and did not exonerate his actions in the eyes of the world. other than him, all the men who were at the conference died during or shortly after the war. the only record of the conference to survive the war was the official editted copy held by the foreign ministry ( i think that's the one) and was the the only written record of how and when the FINAL SOLUTION TO THE JEWISH QUESTION was determined as official government policy. heydrichs offfical orders for the conference were supposedly given by adolph hitler himself.
