Okay....I am revealing the tricks to MY TRADE here....but I hope it helps.
This is a Blankets As Icing Style Cake, 3-Tier
Roll up about 50-60 diapers with rubberbands.
Take a receiving blanket and fold it to the width of the diapers standing up and then secure the two ends together with two safety pins. Set the blanket on a table top so it forms a circle. Start filling that circle with the diapers until it is absolutely stuffed with diapers. Repeat this step with two other receiving blankets....reducing the circumference of the blankets by about three inches each time so that a "tiered effect" is created. Once you have created each tier....remove the diapers one at a time to take the rubber bands off and then re-insert them.
Now you can place items in the middle of the cake by removing diapers to make room for them...this can also help to anchor the tiers together. You may also want to decorate the outside of the cake. Tie ribbon around the bottom tier and then tuck items into that ribbon (bath products, brag books, infant Tylenol, rattles, teethers, etc.). Attach other items with colorful links or diaper pins. Don't forget the plush topper!!
Be creative....think outside the box. I put a link to my site below....check out my "Blankets as Icing" page to get an idea of some finished cakes....
Hope this helped
(shhh...don't tell anyone my secrets!!)

Diaper Cake Recipe
To make you will need:
40-50 newborn diapers (or more depending on how big you want the cake)
2 baby bottles
2 receiving blankets
2 cloth diapers or burp rags
1 large package of various sized rubber bands
ribbon to tie around the cake and hide the rubber bands
Small baby items to decorate the cake
Start by wrapping a diaper, opened lengthwise (but not with the tabs fanned out) around one of the baby bottles. Wrap another diaper around and so on, securing with a rubber band whenever necessary. When you have the desired width for the bottom layer, begin a new layer on top of it, making it slightly smaller than the bottom. Do this again with the top two layers, wrapping around the 2nd bottle. When finished it should look like a four tier wedding cake. Wrap the receiving blankets around the bottom two layers and the cloth diapers or burp rags around the top two. Secure again with a rubber band. Use the ribbon to tie around each layer, hiding the rubber bands. Stick the various baby items inside the ribbon to decorate. Then, add a topper such as a small toy. You may need more or less diapers depending on what size you want the cake.

***84 diapers, pajamas, bibs, bottles, whatever you'd like to include. 2 huge rubber bands. 100 or so regular rubber bands. 15 yards of ribbon on a roll. i made one with 84 diapers. roll each diaper into a tube. use rubber bands for each diaper. 2 large rubber bands. combine the rolled diapers into a circle. cut the individually wrapped diaper rubber band after it is part of the circle. after 3 layers-roll jammies into the same tube shape and size as the diaper. the bottom should be about 14 inches in diameter. 3 inch ribbon works the best to secure the diaper circle layers. on the second layer- do the same thing. roll socks, bibs, and rattles into the second layer. a Precious Moments doll looks great at the top.

If you want to make it yourself why not just go to tesco or any other store that sells them and have a good look at the ones on display and then go home and create one yourself. They are very cute and make a nice center piece. The website some one posted gives the instructions that sound easy and look easy so print out the instructions and still go look in a store to get a clearer picture and then do it yourself
