The above posters that state he has lupus and that is the reason for the scars on his face are correct, as sited in the link to imdb.com. (I wondered myself and looked it up when he became popular).
The reason he got Heidi Klum is he met her at a point when she was vulnerable in her life (pregnant with another man's baby that ditched her) and she has a bit of unconventional personality and is attracted to things that are different (loved wearing black nail polish even before it was popular with goths). How he keeps her is he worships the ground she walks on, gets along well with his in-laws (which are in the mix with helping to raise the children) and a good Dad to all of the collective children.
