You mean when people don't understand one another? Can be the words, can be the non-verbal communication, can be when one person expects a communication and the other assumes that none is necessary. Misunderstandings come in all shapes and sizes. Most are unintentional, but the consequences can be serious e.g. the Battle of Camlan.
