Well technically it's still in the condom but I wouldn't do it.
Theres a good chance that some could slip out. I say once you ejaculate you should get it out...no matter if the woman is unsatisfied.
Just get another condom and try again!

If the condom is still intact, you're about 80% safe but I wouldn't say with 100% truth. Chances are you're OK.

I did that and the girl got knocked up.. so... i think not... she eventually miscarried but it was scary as hell.. it happens cause while your still doing it your thrusting pushes the semen out of the condom and it leaks in.. it's rare but still plausable

yes it wont leak out the penis traps the goo in the condom unless it breaks or is compromised somehow

sure, as long as there isn't a hole the condom will continue to be 93% effective
you should also use the pill to be sure.
